If you need to release or obtain records, please make sure you fill out the release for Sage Health Care. Releases can be obtained by click on “learn more” link located under you provider name. You will see a section that says form and a link to Release of Information. Please fill that out and either email to info@sage-healthcare.com or fax to 208-323-9604.

Psychiatrists

A Psychiatrist is a medical doctor who specializes in the diagnosis and treatment of mental disorders.  Psychiatrists can prescribe medication. Some patients choose to also do therapy in addition to visits with a psychiatrist. Our psychiatrists are all board certified or eligible with the American Board of Psychiatry and Neurology, Inc. Our psychiatrists have many years of experience in treating various aged patients with a variety of mental disorders.
